How CQC inspects services

CQC assesses every health and social care service in England against 5 Key Questions. Each Key Question covers a different dimension of quality and safety, and together they form the framework CQC uses to rate services as Outstanding, Good, Requires Improvement, or Inadequate.

Safe

The "Safe" Key Question asks whether people are protected from abuse and avoidable harm. Inspectors look for evidence of effective safeguarding arrangements, safe recruitment, safe medicines management, safe premises, and a culture where staff feel able to raise concerns. Specific areas inspectors focus on include: how the service manages and learns from incidents; how medicines are administered and stored; whether the right checks were done before staff started work; and whether people feel safe.

Effective

The "Effective" Key Question asks whether people's care achieves good outcomes and promotes a good quality of life. This includes assessment of whether care is based on best practice and evidence; whether people are supported to eat, drink, and maintain their health; and whether staff have the skills and knowledge to meet people's needs. NICE guidance and professional standards are directly referenced in this domain.

Caring

The "Caring" Key Question asks whether staff treat people with compassion, kindness, dignity, and respect. Inspectors observe interactions between staff and people using the service, speak with service users and their families, and look for evidence of a person-centred culture. This includes how staff communicate with people, whether people's privacy and dignity are maintained, and whether people are supported to make their own decisions.

Responsive

The "Responsive" Key Question asks whether services are organised to meet people's individual needs. This includes whether care is person-centred and flexible, whether people can access the service when they need it, and whether complaints are handled well and learned from. End of life care, where relevant, is also assessed in this domain.

Well-Led

The "Well-Led" Key Question asks whether leadership, management, and governance assures the delivery of high-quality, person-centred care. This is often the domain that predicts the overall rating — services with strong governance and a positive culture tend to perform well across all five Key Questions. Inspectors look for evidence of effective quality assurance, engagement with staff and service users, and a culture of continuous improvement.
